Oral surgery for an elderly patient

I care for an elderly dementia patient who has trigeminal Neuralgia and needs several teeth extracted. She will not let anyone near her mouth. She is on narcotic medication for the pain with a course of Pen VK. She is not really a likely candidate for surgery. What can I do to help her pain. Is there any kind of gel, belladonna, or anything we can use to help?
