Most dentist will do the cavity first and repair it. But there are cases where when the dentist starts the treatment and starts to clean out the decay, it can be deeper that what he/she saw on the x-ray. Once the decay is removed, it is very possible that it went below into the root of the tooth. Once that happens, it is most likely that you will need a root canal, as the root will dye on you and can be very painful.

Once you have a root canal done on a tooth, the tooth has no more nerve and it will become brittle and break. That is why a crown is needed after a root canal.
