If your friend is giving you accurate information I would definitely seek a second opinion. After 17 years in the dental field I have never heard of or worked on a patient in which the maxillary front two teeth were removed for Orthodontics. Even if they are unrestorable, meaning they need to be removed and replaced. The space would still be maintained for future restoration. Sometimes depending on the orthodontist, amount of crowding, or occlusion it can be recommended to remove a set of premolars. When this is recommended it is usually performed on both the maxillary and mandibular arches. Removing the front two maxillary teeth would create a large amount of forward movement as well as a total cosmetic restoration of the lateral incisors and canines. As far as removing one lower central incisor, it has been done, however creates a non-existent midline. There are alternatives such as interproximal reduction, distalization, and widening of the buccal corridor that can be done in place of extractions. They are small movements that create a beautiful result. Invisalign uses many of these techniques and is very successful with predictable movements without requiring extractions. I would definitely recommend a second or third option to your friend. Remember once a tooth is removed it cannot be replaced naturally.
