Is apico surgery the right course of action?

Hello! I had a cold sensitive tooth that bothered me for about a year. We tried MI paste, a topical solution, and than replacing the filling. That helped for a few months, but then the sensitivity returned. I had a root canal in Nov 2012, and had "normal" pain afterward. I had a crown put on a week ago. The percussion sensitivity has not changed since then, that is, the crown work did not make the sensitivity any worse. It is now Jan 2013 and I still have percussion sensitivity, and in the past week my right armpit has started to ache. I went to my endodontist this AM and he took more x-rays, said he sees no infection but suspects that the roots are inflamed and that is causing my pain in the tooth and the armpit. He suggested "apico" surgery on the tooth, and if that does not work, an extraction. Does this sound like a reasonable course of action for no visible infection on the x-ray, no fever, but percussion sensitivity and swollen lymph nodes in the armpit? I also have Mitrol Valve Prolapse (with no regurgitation) and no longer have to take meds for dental work, though I did years ago. My endo said that antibiotics would not help in this case and that the options were to go through the crown and retreat the root canal, or do the "apico" surgery. Thank you for your time!! Michelle
